Factores que explican la brecha de cumplimiento de las patologías AUGE del Hospital Regional de Talca

Abstract

El objetivo principal de esta tesis es explicar cuáles son los factores que influyen en la brecha de cumplimiento de las patologías GES en el Hospital Regional de Talca, en específico la patología de Cataratas, esto representa una oportunidad para la poder solucionar los problemas en específicos que aquejan a la patología GES con mayor número de garantías retrasadas en Hospital Regional de Talca. El Hospital Regional de Talca es uno de los tres hospitales dentro de la región del Maule clasificado como hospital de alta complejidad, junto con el Hospital, el Hospital Presidente Carlos Ibáñez del Campo en Linares, Hospital San Juan de Dios en Curicó que concentran el 86% del total de los retrasos en garantía de oportunidad, y específicamente en el Hospital Dr. Cesar Garavagno Burotto el cual concentra la mayor cantidad de patologías retrasadas a nivel regional, con un total de 263, es decir un 82% del total de garantías retrasadas, ya que el Hospital Regional de Talca es el centro de salud final donde se terminan derivando las patologías AUGE tanto para su diagnóstico y tratamiento final dentro de la región y por ende el lugar físico en donde se llevará a cabo la investigación. Las interrogantes para investigar surgen para analizar la gestión que posee el departamento de Oftalmología del Hospital Regional de Talca, lo que se refiere a la planificación estratégica, metas de cumplimiento, modelación a través de Bizagi Moldeler, fichas de procesos, y los factores críticos del proceso. Esta tesis se ha desarrollado de acuerdo a las pautas de investigación exploratoria, y por lo tanto, es de tipo de cualitativa. La principal fuente de información primaria son entrevistas a la coordinadora GES, monitora de la patología GES, Jefe Médico del área de Oftalmología, administrativa de la patología y el encargado de departamento de Planificación y Control del Hospital Regional de Talca, y los conocimientos adquiridos por la observación del proceso de atención de la patología y como fuente secundaria de información se utilizó los documentos del ministerio de salud y de FONASA. Así mismo, se realizó un diagrama de causa-efecto para describir los factores encontrados dentro del proceso de atención de la patología, los cuales se componen de diferentes temas, entre ellos, recursos humanos, tecnológico, estructurales, comunicación, procesos, coordinación. Finalmente, la presente tesis sugiere que de considerarse la identificación de los factores que explican la brecha de cumplimiento de la patología, se deberán plantear futuras investigaciones para implementar un método de gestión. Palabras claves: Planificación estratégica, Factores, Patología, AUGE, Hospital Regional de Talca, Indicador, Cumplimiento Garantías de Oportunidad./ABSTRACT: The main objective of this thesis is to explain what are the factors that influence the breach of compliance of GES pathologies in the Regional Hospital of Talca, specifically the pathology of Cataracts, this represents an opportunity to be able to solve specific problems that they afflict the GES pathology with greater number of delayed guarantees in the Regional Hospital of Talca. The Regional Hospital of Talca is one of the three hospitals within the Maule region classified as a highly complex hospital, together with the Hospital, the President Carlos Ibáñez del Campo Hospital in Linares, San Juan de Dios Hospital in Curicó that concentrates the 86 % of the total delays in guarantee of opportunity, and specifically in the Hospital Dr. Cesar Garavagno Burotto which concentrates the greatest number of delayed pathologies at regional level, with a total of 263, that is 82% of the total delayed guarantees , since the Regional Hospital of Talca is the final health center where AUGE pathologies are derived, both for their diagnosis and final treatment within the region and therefore the physical place where the research will be carried out. The questions to investigate arise to analyze the management of the Ophthalmology Department of the Regional Hospital of Talca, which refers to strategic planning, compliance goals, modeling through Bizagi Moldeler, process cards, and the critical factors of the process. This thesis has been developed according to the exploratory research guidelines, and therefore, it is qualitative. The main source of primary information are interviews with the GES coordinator, GES pathology monitor, Medical Chief of the Ophthalmology area, pathology administrator and the department of Planning and Control of the Regional Hospital of Talca, and the knowledge acquired by the observation of the process of care of the pathology and as a secondary source of information, the documents of the Ministry of Health and of FONASA were used.Likewise, a cause-effect diagram was made to describe the factors found within the process of care of the pathology, which are composed of different topics, including human resources, technological, structural, communication, processes, coordination. Finally, this thesis suggests that if the identification of the factors that explain the pathology compliance gap is considered, future research should be considered to implement a management method.
